"John Macocke [printer] died c. 1692 when this edition was in preparation and this is a possible explanation why all copies appear to be made-up from sheets, or reprinted sheets, from earlier editions. It seems probable that Herringman distributed the title-leaf, the additional leaves and Cutter of Coleman Street to the four booksellers who proceeded to make up copies from sheets in stock"-Perkin
