With an additional title page, engraved, "Gods loue mans vnworthiness" (plate).
With a portrait of the author (plate), signed with monogram of Thomas Cross, in 2 states. State 1, presumably earlier, has almost no shading on forehead between hairline and nose. State 2 has shading in the center of the forehead running down to the nose. The faces in the two portraits are so different as to appear to be different people: state 1 with prominent eyes, nose, and lips, and state 2 with more refined facial features.
